1. No-Refund Policy

All services provided by PreAmptive Ink & Lab Services are non-refundable once rendered. Due to the nature of our services (DNA collection, DOT/Non-DOT urine drug screening collections, notary acts including estate-planning notarizations, ink fingerprinting, I-9 representative services, and life-insurance consultations), the work cannot be “undone” after completion.

No refunds, credits, or chargebacks will be honored after the service has been performed and this form has been signed.

2. Travel Fee

We apply a travel fee to come to you. The amount may vary based on the distance traveled. The travel fee is mandatory, is not applied toward the cost of the service, and is non-refundable once our agent is en route.

3. Cancellation & Refund Policy

• 72+ hours before appointment: Full refund of the service fee and the travel fee, or free reschedule.
• 72 – 48 hours before: 50% refund of the service fee, or one-time reschedule; travel fee refunded in full.
• 48 – 24 hours before: No refund of any kind. One-time reschedule allowed within 30 days.
• Inside 24 hours / same day: All service and travel fees are forfeited in full. One-time reschedule at Company's discretion.
• No-show or 15+ minutes late to the meet-up: All service and travel fees are forfeited in full — no exceptions.
• After service is rendered: No refunds — period.
• Mistreatment of staff: Any threatening, abusive, harassing, or unsafe conduct toward our team voids the appointment with no refund and results in a permanent ban from future service. Authorities will be notified.
• Payments are accepted online only through our secure portal — no cash.

4. Chargeback Warning

Filing a chargeback or payment dispute with your bank or credit card company after receiving services constitutes potential fraud. PreAmptive Ink & Lab Services reserves the right to:

• Submit this signed form, service records, and any photo/video evidence to your financial institution as proof of authorized payment.
• Report fraudulent chargebacks to the appropriate authorities.
• Pursue the outstanding balance through collections or small claims court.
• Refuse future services to the client.

5. Client Acknowledgment

By signing below, I acknowledge and agree that:

• I have read and fully understand the no-refund and cancellation policy above.
• I agree that all services rendered are non-refundable.
• I will not file a chargeback or payment dispute for services I have received and accepted.
• I understand the consequences of filing a fraudulent chargeback as outlined above.
• I am authorizing payment voluntarily and of my own free will.
• I have presented a valid, non-expired U.S. driver's license or government-issued photo ID.
• I am signing this form before any services are performed today.
