Terms of Service

These Terms of Service (the “Terms”) form a binding legal agreement between you (“Client,” “you”) and PreAmptive Ink & Lab Services (the “Company,” “we,” “us”). By visiting this website, requesting a quote, booking an appointment, signing any of our forms, or paying for any service, you acknowledge that you have read, understood, and agreed to these Terms in full and to our Cancellation, No-Show & Refund Policy, Privacy Policy, and Disclaimer. If you do not agree, do not use this website or our services.

1. Acceptance, eligibility & right to refuse service

You must be at least 18 years of age and legally able to enter into a binding contract to book or pay for any service. Services involving a minor require a parent or legal guardian who is present, presents valid photo ID, and signs all required forms.

Our Company expressly reserves the right to refuse, suspend, or terminate service to any person, at any time, for any lawful reason — including but not limited to threatening, abusive, harassing, intoxicated, dishonest, or unsafe conduct directed at our staff, contractors, or other clients. All telephone, text, and email contact with our specialists must remain professional and respectful; our team are human beings with families and we extend the same respect to you that we expect in return. When service is refused under this section, all service and travel fees are forfeited and the client may be permanently barred from rebooking. 2. Services we provide

  • DNA specimen collection — peace-of-mind and chain-of-custody.
  • DOT and Non-DOT urine drug screening collections.
  • Ink fingerprinting on ink cards only.
  • Texas commissioned mobile notary public services.
  • I-9 authorized representative services.
  • Licensed life-insurance consultation and application assistance.

We act as a collection site, mobile notary, and licensed agent only. We are not a clinical or forensic laboratory, a law firm, an immigration consultant, a healthcare provider, an employer, a fiduciary, or a tax/financial advisor. We do not currently offer breath alcohol testing (BAT) and we do not interpret, verify, or release laboratory results — all analysis and reporting is the sole responsibility of the independent SAMHSA-certified laboratory and, for DOT collections, a qualified Medical Review Officer (MRO).

3. Mobile service & travel fee

All services are currently mobile. We come to you. A travel fee applies to every appointment. The travel fee is mandatory, varies based on distance, is subject to change for locations outside our core service areas, does not apply toward the cost of the service itself, and is fully earned and non-refundable once our agent is en route.

4. Identity verification

Every client, donor, signer, and required participant must present a valid, non-expired, U.S. government-issued photo ID and complete the live ID-with-selfie capture step before service begins. No ID, no service — no exceptions, no refunds. We may also require additional supporting documents (e.g., proof of guardianship, employer authorization).

5. Booking, online payment & electronic signature

  • All payments are processed through our secure Square-powered portal. Online payment is required for every service except mobile notary work, where the notarial fees may be paid in person at the time of service — those in-person notary payments are still run through our Square card reader (cards, Apple Pay, Google Pay, or Cash App Pay). We do not accept cash, checks, or money orders for any service.
  • All required intake, consent, HIPAA, I-9, disclaimer, and refund-acknowledgment forms must be signed electronically online before the appointment. The one exception is the chain-of-custody form, which is signed in person at the time of collection as required by laboratory and legal protocol. Paper signatures for the other forms in the field are not a substitute and may delay or void the appointment.
  • A valid, non-expired government-issued photo ID is required at every appointment — even if you completed the online ID upload and electronic forms. No ID at the appointment means no service, and no refund.
  • Bookings are held for twenty (20) minutes pending payment. Unpaid holds are released automatically.
  • You consent to electronic records and electronic signatures under the E-SIGN Act and Texas UETA. Each electronic signature is legally binding and is recorded with timestamp, IP address, and device metadata for evidentiary purposes.

6. Cancellation, no-show & refund policy

Every booking is governed by our Cancellation, No-Show & Refund Policy, which is incorporated into these Terms by reference. In summary:

  • 72 or more hours before: full refund of service fee and travel fee.
  • 72–48 hours before: 50% refund of service fee, or one-time reschedule; travel fee refunded.
  • 48–24 hours before: no refund of any kind; travel fee non-refundable; one-time reschedule allowed.
  • Inside 24 hours / same day: all service and travel fees forfeited in full; one-time reschedule at our discretion.
  • No-show or 15+ minutes late to the meet-up: all service and travel fees are forfeited in full — no exceptions.
  • After service is rendered: all fees are non-refundable, period.
  • Mistreatment of staff: any threatening, abusive, harassing, or unsafe conduct toward our team voids the appointment with no refund, results in a permanent ban from rebooking, and will be reported to the appropriate authorities and, where applicable, to the presiding judge, opposing counsel, employer, or requesting agency.

Refunds, when approved, are issued exclusively to the original payment method within 5–10 business days. We do not issue cash refunds, gift cards, or third-party transfers under any circumstances.

7. Refused or incomplete service

All fees are non-refundable if we are unable to complete a service due to: failure to present valid ID; refusal to sign required forms; suspected adulteration, substitution, or tampering with a specimen; visible impairment or threatening conduct; missing or unauthorized supporting documents; or inability of a signer to demonstrate sound mind, willingness, and awareness as required by Texas notarial law.

8. Truthfulness of information

You represent and warrant that all information, identification, photos, and documents you submit to us are true, accurate, current, and refer to you (or the person you are legally authorized to act for). False, forged, or misleading information voids the service, results in forfeiture of all fees, and may be reported to the appropriate authorities, employer, or requesting party as required or permitted by law.

9. No legal, medical, or financial advice

Information on this website and provided during any appointment is for general informational purposes only and is not legal, medical, tax, or financial advice. Notarization is the act of verifying identity and witnessing a signature — it is not legal review of the underlying document. Life-insurance suitability is discussed during the consultation; nothing on this site constitutes an offer of insurance.

10. Intellectual property

All content on this website — including the PreAmptive Ink & Lab Services name, logo, branding, copy, graphics, and forms — is the exclusive property of the Company and is protected by U.S. copyright and trademark law. You may not copy, reproduce, distribute, scrape, or use it without our prior written permission.

11. Disclaimer of warranties

All services and website content are provided “as is” and “as available” without warranties of any kind, whether express or implied, including (without limitation) warranties of merchantability, fitness for a particular purpose, accuracy, or non-infringement. We do not warrant any specific laboratory, employer, court, or third-party outcome.

12. Limitation of liability

To the maximum extent permitted by law, the Company, its owners, employees, independent contractors, and affiliates shall not be liable for any indirect, incidental, special, consequential, exemplary, or punitive damages, including lost profits, lost data, lost employment, or emotional distress, arising out of or in connection with this website or any service we provide. Our total aggregate liability for any claim shall not exceed the amount you actually paid to us for the specific service giving rise to the claim.

13. Indemnification

You agree to indemnify, defend, and hold harmless the Company and its personnel from and against any and all claims, damages, losses, costs, and expenses (including reasonable attorney’s fees) arising from (a) your breach of these Terms, (b) false or inaccurate information you provided, or (c) your misuse of any service or document we deliver.

14. Chargeback policy

Filing a chargeback or payment dispute for a service that was rendered, a no-show that was forfeited, or any fee that is non-refundable under these Terms or our Cancellation Policy constitutes a material breach of this agreement. We will contest such chargebacks with full documentation — including signed forms, timestamps, IP records, and dispatch logs — and you agree to be responsible for any chargeback fees, collection costs, and reasonable attorney’s fees we incur.

15. Governing law & venue

These Terms are governed by the laws of the State of Texas, without regard to its conflict-of-laws principles. You and the Company consent to the exclusive jurisdiction and venue of the state and federal courts located in Tarrant County, Texas, for any dispute arising out of or relating to these Terms or any service.

16. Severability & entire agreement

If any provision of these Terms is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect. These Terms, together with the Cancellation Policy, Privacy Policy, Disclaimer, and any service-specific consent or acknowledgment form, constitute the entire agreement between you and the Company and supersede all prior agreements, oral or written, on the same subject.

17. Updates

We may update these Terms from time to time. The version in effect on the date you booked your appointment is the version that governs that appointment.
