PAYMENT NOTICE: DOT and Non-DOT testing is paid in full online before the appointment through Square. If mobile service is requested, the travel fee is also collected upfront and is non-refundable once our agent is en route. GOVERNMENT-ISSUED ID REQUIRED: The donor must upload and present a valid, non-expired U.S. driver's license or government-issued photo ID at the time of the appointment — no exceptions. PreAmptive Ink & Lab Services acts as a collection site only. We do not currently offer breath alcohol testing (BAT), nicotine/cotinine testing, or K2/synthetic cannabinoid (Spice) testing. We do not interpret, verify, or release laboratory results. All specimens are analyzed by an independent SAMHSA-certified laboratory and (for DOT testing) reviewed by a qualified Medical Review Officer (MRO). Turnaround time, chain-of-custody disputes, and final result determinations are the responsibility of the lab and MRO. Results are sent by the laboratory via email. Any prescriptions or medications the donor is currently taking must be disclosed on the intake form so the MRO can contact them if questions arise. A refusal to test, an adulterated/substituted specimen, or failure to provide a sufficient specimen will be reported to the employer/requester as required by 49 CFR Part 40 (DOT) or applicable non-DOT policy. By booking, the donor and employer agree that our liability is strictly limited to the collection fee paid.
